Official Rules to the Grand Slam Golf Giveaway
NO PURCHASE NECESSARY TO ENTER OR WIN: The Grand Slam Golf Giveaway (the "Promotion") begins at 12:00 Noon Eastern Time on 7/1/2008 and ends at 5:29 PM Eastern Time on 12/31/2008.
ELIGIBILITY: The Promotion is open to all who are 21 years of age or older at the time of entry. (Upon verification of prize eligibility, misrepresentation of age will void the entry.) Employees and directors of the Florida's First Coast of Golf, Inc. and their respective parents, subsidiaries, and contractors or advertising/promotional agencies who are directly or indirectly involved are not eligible to enter or win this Promotion; also, members of their immediate families (parents, children, siblings, spouse) and persons living in the same households as such individuals (whether related or not) are not eligible to enter or win this Promotion.
HOW TO ENTER: Enter this Promotion online at (www.florida-golf.org/six-night-golf-giveaway/). To enter, complete the online form with your name, unique e-mail address and zip code.
After entering the correct information in all required fields of your initial (unique) entry, you can click on the "Submit Entry " button. A confirmation email will be sent the the email address you submitted. Your entry will then be entered into the Florida's First Coast of Golf, Inc. database for this Promotion. Subsequent entries from the same person using the same email address will be disqualified. Any attempt by a person to enter more than permitted with the same email account or with impermissible referral e-mail addresses will result in disqualification of the entrant at the discretion of the Florida's First Coast of Golf, Inc. upon verification. Electronic entries will be deemed made by the authorized account holder of the unique e-mail address submitted at time of entry. Odds of winning depend on the number of eligible entries received.
PRIZE:One (1) Grand Prize. Winner and one (1) guests selected by the Winner and qualified by the Sponsor (the "Guests") will enjoy The Grand Slam Golf Giveaway between 7/1/2008 and 12/31/2008.
Upon award of the prize, Winner and Sponsor will select mutually agreeable dates between 7/1/2008 and 12/31/2008 to take the Prize. Winner's Guests must be 21 years old by redeemed date, unless accompanied by a parent or legal guardian.
Prize includes: one (1) deluxe guest room (double occupancy) for two (2) nights at each of the lodging facilities mentioned in the prize package; two (2) greens/cart fees for a round of golf for two (2) at each resort mentioned in the prize package. Winner must transfer to each resort in the package and may not extend beyond the 2 night stay at each resort. Approximate Retail Value (ARV) is $2,400 depending on dates selected.
Prize does not include: airfare; spending money; transportation to and from the airport, hotel or golf courses; meals; gratuities or service charges. Winner is responsible for all federal, state and local income taxes on prize, and any expenses related to the acceptance or use of the prize not specifically described as part of the Prize in these rules.
Prize is not transferable. No prize substitution or cash equivalent of prizes permitted, except at the sole discretion of Sponsor. Accommodations and golf are based on availability at all award properties.
SWEEPSTAKES DRAWING AND NOTIFICATION OF WINNERS: Winner will be determined by random drawing from all eligible entries on or about 1/10/2009 by the Florida's First Coast of Golf, Inc.. One (1) prize will be awarded to one (1) person for this Promotion, upon winner verification and final approval. The selected winner will be notified by e-mail, and/or telephone or mail. Selected winner will have five (5) business days to respond to the notification via e-mail, and/or telephone or mail or may be disqualified at the discretion of sponsor. Upon receipt of affirmative response, The Florida's First Coast of Golf, Inc. will send an Affidavit of Eligibility, Liability Release and Publicity Release. Selected winner will have ten (10) days from the date the Florida's First Coast of Golf, Inc. sends the documents to claim the prize by completing, signing and returning the Affidavit of Eligibility, Liability Release and, where legal, Publicity Release. If selected winner cannot be contacted, is ineligible, or fails to timely return the completed and executed Affidavits and Releases as required, an alternate will be selected randomly from among all remaining eligible entries until the prize is awarded.
Once confirmed, the Prize Winner must provide the Florida's First Coast of Golf, Inc. with the names, e-mail address (if available), street address, and telephone number of the person he/she has selected for the as Guest within fourteen (14) days. the Florida's First Coast of Golf, Inc. will send the potential Guest an Affidavit of Eligibility, Liability Release and, where lawful, Publicity Release form. The designated Guest must properly complete and return these documents to the Florida's First Coast of Golf, Inc. within seven (7) days, or be subject to replacement. Winner shall have at least one more opportunity to nominate a Guest. If the Florida's First Coast of Golf, Inc. has not received satisfactory completed information for the one (1) qualified Guest within twenty (20) days, it shall notify Winner by email and Winner shall forfeit that portion of the prize unless Sponsor, in its sole discretion, finds it can allow additional time for Guest qualification. All decisions of the Florida's First Coast of Golf, Inc. shall be binding and final.
OTHER TERMS: The Promotion is an electronic random drawing. By entering the Promotion, you agree to release, discharge and hold harmless the Florida's First Coast of Golf, Inc., all participating Partner Sites and their respective parents, affiliates, subsidiaries, contractors and advertising/ promotional agencies and their respective officers, directors, employees and agents, from any and all damages whether direct or indirect, which may be due to or arise out of participation in the Promotion or the acceptance, use/misuse or possession of the prize.
By participating, you agree to these Official Rules and to the decisions of the Florida's First Coast of Golf, Inc. judges, whose decisions are final and binding in all respects. Acceptance of a prize constitutes permission for the Florida's First Coast of Golf, Inc. and participating Partner Sites to use Winner's name and likeness for advertising and promotional purposes without compensation, unless otherwise prohibited by law. Void where prohibited by law, rule or regulation. All federal, state and local laws and regulations apply. All information collected will be treated in a manner consistent with the Florida's First Coast of Golf, Inc. privacy policy.
In the event any portion of this promotion is compromised by a computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures or any other cause beyond the control of the Florida's First Coast of Golf, Inc. which, in the sole opinion of Florida's First Coast of Golf, Inc. corrupts or affects the administration, security, fairness or proper play of the Promotion, Florida's First Coast of Golf, Inc. reserves the right to suspend or terminate the Promotion and award the prize to a winner randomly selected from among entries received prior to the compromise.
The Florida's First Coast of Golf, Inc. and Partner Sites are not responsible for lost, interrupted or unavailable network server or other connections, miscommunications, failed telephone or computer or telephone transmissions or technical failure, jumbled, scrambled or misdirected transmissions, traffic congestion on the Internet or at any web site, nor other error of any kind whether human, mechanical or electronic, including any injury or damage to participant's or any other person's computer relating to or resulting from participating in this Promotion or downloading any materials in this Sweepstakes.
Persons found tampering with or abusing any aspect of this promotion, as solely determined by the Florida's First Coast of Golf, Inc., will be disqualified. If disqualified for any of the above abuses, the Florida's First Coast of Golf, Inc. reserves the right to terminate player's eligibility to participate in the Promotion. the Florida's First Coast of Golf, Inc. is not responsible for any problem with a Promotion entry generated by computer hardware or software malfunction, error or failure. the Florida's First Coast of Golf, Inc.'s sole responsibility for any irregular Promotion entry is replacement with another entry. In the event of a dispute as to any entry, the authorized subscriber of the email account used to enter the Promotion will be deemed to be the entrant.
Potential winner may be required to show proof of being the authorized account holder. (The authorized account holder is the natural person assigned an e-mail address by an Internet access provider, online service provider or other organization responsible for assigning e-mail addresses for the domain associated with the submitted address.) Entries will not be acknowledged.

GENERAL INFORMATION: Entrant agrees that any and all disputes will be resolved in the appropriate state or federal court located in the State of Florida, Duval County, and be subject to the laws of the State of Florida. Should there be a conflict between the laws of Florida and any other laws, the conflict will be resolved in favor of the laws of the State of Florida. This promotion is conducted in English.
